The IOM report, “Crossing the Quality Chasm”, called
for fundamental reforms in the health care system. It
identified deficiencies and problems that contribute to
sub-optimal health care quality, compromised patient safety,
and waste in the system. More importantly, it outlined an
agenda for making our health care system safe, effective,
patient-centered, timely, efficient and equitable.
The
