RELAX!

Flexibility is the cornerstone of "going green", whether we are talking about personal habits or technology. Let's consider these "5 Rs" through the eyes of the Statit Solutions Group with a request that is heard often. Many healthcare organizations are very familiar with The Joint Commission's OPPE initiative. Understanding physician performance is a critical component in continuous process improvement initiatives. Statit Solutions is a leading provider of Physician Profiling solutions. Let us now consider a variation of this theme; allowing us to conquer the first "R"…reduce spending.

Consider the effect a scorecard with key performance indicators would have on your CEO and his/her understanding of your business. The scorecard would include "traffic light status" relative to targets and alarms together with comparisons to other peer facilities, departments, units, etc. Also consider providing the CEO with drill-down capabilities into the details of each indicator. Let's continue…how about providing the CEO with an ability to schedule and interactively review in written format the performance of each facility right from his/her scorecard view. Timely feedback is an important step toward resolving and/or encouraging behavior. Finally, envision a centralized quality portal where all types of key performance indicators comprised of financial, patient satisfaction, human resources and other strategic objectives meld together in one place…consistently represented and of course, fully automated. Let's consider the following examples and output from the CEO perspective.

Based on the credentials of the user, the scorecard displayed is specific to that user with associated permissions (drill downs—yes or no, comparisons— blinded vs. unblinded, record review notes—yes or no, etc.). Let's go another step forward and begin peeling back the onion. The next shots capture the live link information from the Average Length of Stay in Days indicator and associated drill downs.

For brevity purposes, the drill-down table from Albany General Hospital has been cropped. The table contains a wealth of information in a summarized view for each patient, including LOS. The Details link brings the CEO directly into the patient record.

Finally, let's close the loop. The CEO can review the performance of each facility against targets, compare facility performance against peer group (bed size, region, etc.) and provide review notes and schedule the next review.

Using these same indicators (renew and recycle), we can then create a profile for the facility manager. Given the permissions of the facility manager, Statit Solutions return measurements and analyses for their facility against blinded comparisons to other facilities in the delivery system and drill down into his/her own facility's data. The facility manager can choose to review units or departments within each facility for comparison purposes. The facility manager can then close the loop by providing reviews on each unit or department. Finally, the facility manager can schedule when the next review will take place, all on-line and available to the staff of the facility to see and understand.

Finally, the people who really make all of this good take place…the staff members! When a staff member logs in, they will see how their particular unit or department is performing, together with the reviews written by the manager.

Continuous process improvement at its best! One indicator shared throughout several levels of personnel but done so with different "views" of the data. One of the most fundamental questions to be addressed to improve performance is deceptively simple: How do we know when health care services are "good"? The answer, also simple, is to measure those services. Provide each stakeholder with measurement of the current situation and objectives for the future and not too surprisingly, your chances for success will be greatly enhanced!
